continuing this Volunteer Connection Newsletter, now bi-monthly, to provide
organization and procedural updates, as well as educational and connection
opportunities. See what you think! Feel
free to share your opinion. This is your newsletter. Make it interactive! If it
isn’t providing something you are interested in or need, please let us know!

v HELP
WANTED: To meet patient demand, we need to
increase our volunteer pool near Thornton, Brighton, Westminster and
Arvada. Please let Wendy know of any organizations or contacts you think may be
helpful in this effort.
v DOCUMENTATION:
Reminders about Requirements
Hospice Assignments:
Ø Most assignments
are ONE Hour, Weekly Visits
(Respites, Life Reviews, Errands
excepted.)
Ø Weekly visits
require Weekly documentation; even if you haven’t met with pt. (Consider this
your Golden Thread of information.) Simply update with reason for no visit or
no contact and provide expected next step or visit.
Ø Please don’t wait
for patients or families to call you, even if that is what they request. A
simple weekly check in call will allow you to document and update the Team.
Please remember families are in crisis and may forget to utilize your support.
Ø Please document observable
details. Refrain from indicating ‘Patient is As Usual.’
Ø Please document
your full name and the patient name as indicated on the assignment. “Chuck” he
may be to you but his medical record recognizes him as “Robert Redford.” (If
you are still reading, you are welcome for that smile.)
Palliative Care Assignments:
Ø Noted on the
Request Board as Palliative Assignments, these are biweekly, as are the
expected documentation.
Ø Palliative
patients are not as closely monitored by Team members. You may have more
insight. Please share as timely as possible. We have asked the Palliative Team to
provide the same, with more timely updates.
