Healthcare Values and Zoom Schedule

The Conversation Project: Talking about Healthcare Values

Zoom

City of Louisville Recreation and Senior Center

Tuesday, August 10th at from 1-2pm

Free

92% of Americans say that it is important to have conversations about healthcare and end-of-life wishes, but only 32% have actually had them. If loved ones do not know what matters most to you, they may be left with distress and uncertainty. Starting these conversations is never easy – but families and health care professionals report feeling better prepared when the subject is addressed. Advance care planning can help ensure that your wishes are heard and respected.

Join The Conversation Project in Boulder County for an overview of advance care planning, focused on open communication and “the conversation.” The future is unpredictable, but we can help you prepare those that matter most be prepared to advocate for you in a variety of circumstances.

1)
Watch this 40 minute COVID education refresher. It will tell you everything you need to know to resume in-person visits. There is a section about weekly testing. This can be ignored. All other requirements mentioned in the video are still in place.

Complete the COVID Competency exam and return it to us at volunteer@trucare.org or by regular mail to the address in my signature below.

3) Complete this short

4)
Complete a personal health screening before each visit or before each shift at the Care Center. For home visits, call the family the day of the visit to confirm they are prepared to see you. Ask them: "has anyone in the home had symptoms of COVID-19 in the last 7 days."  If they say yes, cancel your visit and let us know. Indicate their answer on the corresponding question on your personal health screening.
